COVID-19 Vaccination Mandate

Individuals covered under this policy include all full-time, part-time and PRN employees of AVALA Hospital as well as all licensed practitioners including physicians, vendors, allied health professionals, contractors, contract workers, volunteers, interns, externs, and any other individual who provides care, treatment or other services or who has contact with those individuals who perform such services at AVALA Hospital.

Employees of AVALA Physician Network (APN) are not required to be fully vaccinated with the exception of Physicians who perform procedures at AVALA hospital, any employee that assists the physician with procedures at AVALA Hospital or provides patient care, and any Physical Therapy or Occupational Therapy employees associated with APN (collectively “Covered Individuals”).

All Covered Individuals are considered “fully vaccinated” two (2) weeks after completing primary vaccination with a COVID-19 vaccine, with, if applicable, at least the minimum recommended interval between doses. For example, this includes two weeks after a second dose in a two-dose series, such as the Pfizer or Moderna vaccines, or two weeks after a single dose vaccine such as the Johnson and Johnson vaccine.

Covered Individuals returning from leave of absence and new hires on or after November 15, 2021 are required to comply with this policy by January 27, 2022 or upon date of hire and as a condition of employment.

Effective November 15, 2021, all new hires and re-hires with AVALA Hospital who are Covered Individuals must provide acceptable proof of COVID-19 vaccination status or make a medical or religious exemption (or medical deferral) request by January 27, 2022 or upon date of hire or for employees returning from leave, upon return to work from leave. Exemption forms are available upon request from Human Resources.
