Please note: The charges listed reflect AVALA facility charges only. They do not include fees billed separately by physicians, anesthesia providers, radiologists, laboratory services, pharmacy, or other professional service groups. Actual patient responsibility will depend on insurance coverage and benefit structures, which differ widely from person to person.

If You Have Commercial Insurance

• Contact your insurance company to understand your benefits and the factors that affect your financial responsibility.

• Your out-of-pocket costs depend on plan type, deductible, co-insurance, co-pays, out-of-pocket maximums, provider network status, and plan limitations.

• Prices listed on this website do not include professional fees such as physician services, imaging, lab work, or pharmacy charges.

• Prices also do not reflect any negotiated discounts between AVALA and your insurance company.

If You Have Medicare

• Medicare coverage varies by beneficiary, as not everyone has the same Medicare plan or supplemental coverage.

• Medicare may pay for many healthcare services, but not all.

• Special rules apply if you have employer group health coverage through your job or a spouse’s job.

• Supplemental insurance may cover some costs that Medicare does not.

If You Have Medicaid

• Contact the Louisiana Medicaid office at 1-888-544-7996 or visit the Louisiana Department of Health website to understand coverage details.

• In most cases, Medicaid recipients are not responsible for any portion of the bill.

• Please contact our AVALA Patient Financial Services Department at (985)809-9888 for information regarding payment policies and available options.

• As required by the Affordable Care Act (ACA), AVALA maintains a Financial Assistance Policy that outlines eligibility criteria, how charges are calculated, and the application process.

• Our Patient Financial Services team can help determine whether you meet the criteria for financial assistance.
