HOW TO REQUEST MEDICAL RECORDS?

As a patient and or patient representative, it’s your right to obtain a copy of your medical records. AVALA partners with MRO to provide you with a copy of your medical records in an easy, yet private and secure manner. With your request we can provide your medical records through the following:

Access Your Medical Records through the AVALA Patient Portal

You can sign up to view your or your child’s medical record from your computer or smartphone using the AVALA Patient Portal.

This portal enables you to view imaging reports, visit summaries, clinical notes, your health profile, procedure summaries, family and social history, review of upcoming appointments, medication refill requests, or the messaging center to speak to your provider; however, there are some limitations to what can be viewed in the AVALA Patient Portal.

Things to Know

All requesters must provide proof of identification or documented proof of legal relationship as appropriate (personal representatives, patient advocate, executor, trustee or guardian) if requesting medical records on behalf of a patient, as well as identification, along with the medical record request form.

• Patient Authorization
  MRO keeps records of the care and services you receive at our facility. Your health information is personal and we are committed to keeping it confidential. We are required by law to respect your privacy.
  
  
• Processing Time
  MRO is dedicated to providing you with your records in a timely manner. Medical record requests are processed in the order they are received and will be delivered to you as quickly as possible. For urgent needs, please contact the medical records department directly by emailing him@avala.com or calling 985-809-9888.
  
  
• Fees for Records
  Depending on the purpose of your request, you may incur a fee for a copy of the records. You will be advised of any potential fees when your request is submitted and again before it is completed.
