Covington, Louisiana – AVALA is happy to announce that Becker’s Healthcare has named them one of the nation’s Top Hospitals for Care Transitions. A care transition is a process of moving from one level of care to another, such as from a hospital to a home. This recognition shows that AVALA stood out among all facilities nationwide for providing patients with some of the best post-op care they can receive.
AVALA takes pride in the internal teams that make patients’ transition to home very successful and as seamless as possible. The team sends patients a beautiful educational booklet that tells them exactly what to expect before and after their procedure and contact information for those who can help with additional questions.
The team’s goal is never to let a patient or their caretaker leave with any questions about their post-procedure care and maintenance. Our team creates a wealth of information for patients to bring home and continue their care, such as a discharge plan with detailed instructions for use at home and a guide for incision site management.

CMS shares 10 HCAHPS star ratings based on publicly reported HCAHPS measures. The care transitions star rating combines data from three HCAHPS survey questions:
- During this hospital stay, staff took my preferences and those of my family or caregiver into account in deciding my healthcare needs when I left.
- When I left the hospital, I understood the things I was responsible for in managing my health.
- When I left the hospital, I clearly understood the purpose of taking each of my medications.
The star rating is based on survey data collected from hospital patients from July 2021 through June 2022. The figures are from CMS’ Provider Data Catalog and were released on April 26. Asterisks denote that CMS included a footnote about the organization’s data, summarized below.
